RESUME

Introduction: La dose de dialyse est un concept relativement  récent. Il permet de juger la qualité d’épuration des toxines urémiques pour garantir au patient dialysé chronique une meilleure survie. Le Kt/V urée, malgré ses imperfections reste un indicateur reconnu comme le plus pertinent cliniquement à évaluer. Il peut être aisément calculé en ligne grâce à l’équipement des moniteurs de dialyse avec la dialysance ionique. Objectif : L’objectif de cette étude est de déterminer la corrélation entre Kt/Vocm (online clearing monitoring), et les autres méthodes de mesure directe : Kt/Vsp single pool, Kt/Ve équilibré et PRU pourcentage de réduction de l’urée. Matériel et méthodes : Etude transversale observationnelle mono-centrique chez des 50 patients hémodialysés chroniques. Durant les mêmes séances, nous avons mesuré le Kt/Vocm et calculé le Kt/Vsp, Kt/Ve et le PRU en pratiquant des prélèvements sanguins avant, après et 15 minute après chaque séance, afin de déterminer la corrélation entre le Kt/Vocm et les autres méthodes de mesure. L’analyse statistique a été effectuée par le logiciel statistique SPSS 17.0. Résultats : Une corrélation positive, statistiquement significative a été retrouvée entre Kt/Vocm et les autres méthodes : Kt/Vsp(r=0,55 ; p < 0,001), Kt/Ve(r=0,58 ; p < 0,001), et PRU(r=0,54 ; p < 0,001). Conclusion : notre étude suggère que le monitorage automatique de la dialysance ionique parait être une méthode facile et fiable pour la mesure de la dose de dialyse.

ABSTRACT

Introduction: The dialysis dose is a new concept that provides the quality assessment of purification of uremic toxins during hemodialysis session (HS). Several reports have showed the benefit of such measurement to improve the survival of maintenance hemodialysis patients (MHP). Although some imperfections, Kt / v urea, remains a well-recognized tool for dialysis dose control. The objective of this study is to evaluate the correlation between different measurement tools of dialysis dose. in a group of 50 MHP. Material and methods: the study design is Mono-centric, observational and cross-sectional. The dialysis dose assessment tools are: Online clearance monitoring kt/v (ocm kt/v), a method that provide an online Kt/v, performed by the dialysis machine using ionic dialysance and the other Kt/v measurement methods: Single pool kt/v (sp kt/v), equilibrated kt/v (ekt/v) and the percentage of urea reduction (PUR) by using blood samples before, at the end and 15 minutes after each hemodialysis session. Statistical analysis was performed by SPSS 17.0 statistical software. Results: fifty MHP have been enrolled. A positive, statistically significant correlation was found between ocm kt/v and the other methods: sp kt/v (r = 0.55; p <0.001), ekt/v (r = 0.58; p <0.001), and PUR (r = 0.54; p <0.001). Conclusion: our study suggests that automatic monitoring by ionic dialysance seems to be an easy and reliable method for measuring the dialysis dose.
